THIS CONTRACT IS BETWEEN THE CITY OF PEMBROKE PINES/SOUTHWEST FOCAL POINT SENIOR CENTER AND AREAWIDE COUNCIL ON AGING OF BROWARD COUNTY, INC. TO PROVIDE OLDER AMERICANS ACT IIIB & IIIE SERVICES FOR THE PERIOD OF JANUARY 1, 2018 TO DECEMBER 31, 2018.

SUMMARY EXPLANATION AND BACKGROUND:
1. The 2018 Application was submitted to the Areawide Council on Aging of Broward County, Inc on September 6, 2017 with Agenda 17-0579.
2. The 2018 Application was approved and Contract JA118-10-2018 was awarded. Funds received were allocated to $329,464.70 Federal Funds OAA Title IIIB, $48,867 Areawide Council on Aging Local Match for Fund IIIB, $88,728.20 Federal OAA IIIE Funds, $95,284.00 City Contribution In-Kind.
3. Amendment 001 to Contract JA118-10-2018 represents additional Federal Funds in the amount of $47,810.00 to Funds IIIB & IIIE. Services offered under this Contract are: Adult Day Care, Counseling Individual and Group, In-Home Services (Homemaker, Personal Care and Respite), and Recreation for our senior population 60 year of age and older living in the Southwest Sector of Broward County.
