MOTION TO APPROVE SUBMISSION OF THE APPLICATION FOR THE GENERAL REVENUE CONTRACT - LOCAL SERVICE PROVIDER (LSP) FOR THE FISCAL YEAR OF JULY 1, 2020 TO JUNE 30, 2021 IN THE AMOUNT OF $231,155.00. THESE FUNDS ARE FROM THE FLORIDA DEPARTMENT OF ELDER AFFAIRS PASSED THROUGH THE AREAWIDE COUNCIL ON AGING OF BROWARD COUNTY TO PROVIDE TRANSPORTATION SERVICES TO OUR SENIOR CLIENTELE.

SUMMARY EXPLANATION AND BACKGROUND:
1. The current Contract JL119-10-2020 between the City of Pembroke Pines/Southwest Focal Point Senior Center and, the Areawide Council on Aging of Broward County was approved at the Commission Meeting of June 19, 2019 with Agenda Item #19-0737. The term of this contract is from July 1, 2019 to June 30, 2020 in the amount of $231,155.00.
2. This application represents continuation of Transportation Services to our Senior Clientele 60 years of age and older. Among the services offered are portal to portal, to and from nutrition program, grocery shopping, medical appointments and, other destinations to meet the individual need. These services preserve elder independence, support caregivers, and target at risk persons.
3. Approximately 24,776 units of transportation services will be provided to our senior population.
4. Requesting Commission approval for the submission of the General Revenue Application LSP FY2020-2021.
Item has been reviewed by the Commission Auditor and approved for the Agenda.
